Forms for the 2023-24 Academic Year
These forms may be requested by our office or could be optionally submitted for a reconsideration of financial aid eligibility. Carefully read the information on the form, provide all requested supplementation documentation (as applicable) and be mindful of the signature certifications. After initial review of your submission, additional documentation and/or clarification may be requested.

Commonly Requested Forms
- Verification Worksheet - Dependent Student (PDF) - For Dependent Students selected for Verification.
- Verification Worksheet - Independent Student (PDF) - For Independent Students selected for Verification.
- Outside Scholarship/Resources Reporting Form (PDF) - Notify Financial Aid of any scholarships or resources you have received from organizations or persons outside William & Mary.
- 2023-24 Graduate Student Enrollment Worksheet (PDF) - Report anticipated enrollment to our office. This ensures that the financial aid package we create accurately reflects your plans and costs.

Additional Financial Aid Forms
- Appeal for Special Circumstances Consideration (PDF) - Report unforeseen changes in family financial situations not addressed by the FAFSA filing.
- Computer Expense Request Form (PDF) - This form is used to request the cost of a new computer be included in the student's Cost of Attendance. This request will allow for additional loan eligibility, either student or parent loan depending on the student's existing financial aid package.
- Dependency Override Form (PDF) - Only submit this form after consulting with your financial aid counselor.
- Dependency Status Worksheet (PDF) - Only submit this form if requested. This form is used to clarify a student's response to questions 46 - 54 on FAFSA.
- Dislocated Worker Verification Worksheet (PDF) - Only submit this form if we have specifically requested it.
- Homeless Unaccompanied Youth Determination Form (PDF) - This form is used to clarify a student's response to questions 55-57 on FAFSA.
- Identity and Statement of Educational Purpose (PDF) - Only submit this form if we have specifically requested it.
- Monthly Budget Worksheet (PDF) - Only submit this form if we have specifically requested it.
- Original Citizenship Document Affidavit (PDF) - Students required to submit citizenship documents should present them in person to the Office of Financial Aid. When it is impossible to come in person, complete this form and mail legible copies of your citizenship documents to our office. DO NOT submit this form if you can bring your original document to the Financial Aid Office.
- Sibling Enrollment Verification Form (PDF) - Certify a W&M Student's sibling enrollment in a college/university program at least half-time. Only submit this form if we have specifically requested it.
- Summer Financial Aid Application - Undergraduate Students (PDF) - W&M Undergraduates planning to attend summer school at W&M need to submit a summer financial aid application to inform our office so that financial aid eligibility can be updated.
