Forms for the 2024-25 Academic Year
These forms may be requested by our office or could be optionally submitted for a reconsideration of financial aid eligibility. Carefully read the information on the form, provide all requested supplementation documentation (as applicable), and be mindful of the signature certifications. After an initial review of your submission, additional documentation and/or clarification may be requested.

Commonly Requested Forms
- Outside Scholarship/Resources Reporting Form (PDF) - Notify Financial Aid of any scholarships or resources you have received from organizations or persons outside William & Mary.
- 2024 - 25 Graduate Student Enrollment Worksheet (PDF) - Report anticipated enrollment to our office. This ensures that the financial aid package we create accurately reflects your plans and costs
Additional Financial Aid Forms
- Appeal for Special Circumstances Consideration (PDF) - Report any unforeseen changes in family financial situations not addressed by the FAFSA Filing.
- Dependency Status Worksheet (PDF) - Only submit this form if requested. This form is used to clarify a student's response to the Student Personal Circumstances Section on FAFSA (Section 5).
