The OGS is transitioning forms from PDF to DocuSign. For the most up-to-date versions of OGS forms, only use those found on this page and on the Quick Links page. We will phase out the old PDF forms in batches beginning on March 18, 2024.
Forms now in DocuSign must be submitted in DocuSign. Any remaining PDF forms must be continue to be submitted via W&M email address to dean-gsr@wm.edu.
