Forms for the 2026 - 27 Academic Year

These forms may be requested by our office or could be optionally submitted for a reconsideration of financial aid eligibility. Carefully read the information on the form, provide all requested supplementation documentation (as applicable), and be mindful of the signature certifications. After an initial review of your submission, additional documentation and/or clarification may be requested

Commonly Requested Forms

• Verification Worksheet - Dependent Students (PDF) - For Dependent Students selected for Verification. 
• Verification Worksheet - Independent Student (PDF) - For Independent Students selected for Verification.
• Outside Scholarship/Resources Reporting Form (DocuSign) - Notify Student Accounts & Financial Aid of any scholarships or resources you have received from organizations or persons outside William & Mary. This form is also used to request that W&M Student Accounts send an invoice to scholarship donors. Please be aware that this form will require the student to log in with W&M credentials to access.
• 2026 - 27 Graduate Student Enrollment Worksheet (PDF) - Report anticipated enrollment to our office. This ensures that the financial aid package we create accurately reflects your plans and costs. 
  • Graduate Students who are starting their program at W&M in Spring 2026 and Summer 2026 should complete the 2025 - 2026 Graduate Student Enrollment Worksheet
• 2026 - 27 Undergraduate Student Enrollment Worksheet (PDF) - Report anticipated enrollment to our office. This ensures that the financial aid package we create accurately reflects your plans and costs.

Additional Financial Aid Forms

• Appeal for Special Circumstances Consideration (PDF) - Report any unforeseen changes in family financial situations not addressed by the FAFSA Filing. 
• 2026 - 27 Asset Verification Worksheet (PDF) - Used to verify current assets for FAFSA filers
• Computer Expense Request Form (PDF) - This form will be available for submission 60 days prior to the start of the Fall 2026 semester on June 27, 2026.  This request may potentially increase loan eligibility, either for a student loan or a parent loan, depending on the student's existing financial aid package. A student may also indicate that they are requesting a cost increase only to allow for additional room for anticipated outside scholarships/resources in their package.
• 2026 - 27 Dependency Status Worksheet (PDF) - Only submit this form if requested. This form is used to clarify a student's response to the Student Personal Circumstances Section on FAFSA (Section 5).
• 2026 - 27 Dependency Override Worksheet (PDF) - used by students who are applying for an exceptional circumstances appeal for independent student status for the first time.
• 2026 - 27 - Parent Refusal to Provide FAFSA Information (PDF) - used when a student has indicated in their FAFSA filing that their parents are refusing to provide FAFSA information and they want to be considered for Unsubsidized Student Loan eligibility
• Identity Verification Worksheet(PDF) - Only submit this form if we have specifically requested it. This form must be mailed or completed in person; it cannot be accepted by fax or digital upload.
