Release of Information

There is a fee of $1.00 per page associated with requests of copies of records. This does not include requests for immunization records. The Student Health Center only accepts VISA, Mastercard, Discover, American Express, or your W&M Express (we do not accept cash or checks). This information can be called in directly to the Student Health Center personnel at (757) 221-4386 rather than including it on your faxed Release of Information.
Any student who is no longer attending William & Mary should include the date she/he last attended the university. Please note, records are only kept for 6 years following your graduation date or last date attending W&M.
If a student has been away from the university for an extended period of time, the medical record has been archived, and it will take approximately one week to retrieve the record from archives.
If you are picking up your records at the SHC, you will need to present your Student ID card to the front desk.
