Immunization Consent

The administration of vaccines to minors requires parent/guardian consent. To proceed with scheduling a vaccination appointment, please follow these steps:

1. Download the Vaccine Administration Parental Consent Form (pdf).
2. Have your legal custodial parent or legal guardian review the appropriate Vaccine Information Statement(s) and complete the consent form.
3. Upload the completed form to your Online Health Portal.
4. A member of the Student Health Center team will reach out to schedule an appointment for administration of the vaccine(s).
