Immunization Consent
The administration of vaccines to minors requires parent/guardian consent. To proceed with scheduling a vaccination appointment, please follow these steps:
- Download the Vaccine Administration Parental Consent Form (pdf).
- Have your legal custodial parent or legal guardian review the appropriate Vaccine Information Statement(s) and complete the consent form.
- Upload the completed form to your Online Health Portal.
- A member of the Student Health Center team will reach out to schedule an appointment for administration of the vaccine(s).
