Join a Sport Club

There are several required items that must be submitted within 3 business days of your first practice or event. Athletic Trainers and/or sport club staff must approve your forms before you're cleared to play.

Healthy Roster

First-time users can create a Healthy Roster profile after receiving an email invite to Healthy Roster and then following the directions in the email you will receive from Healthy Roster or the How-To Document. If you are a first-time sport club member or a returning member who does not have a Health Roster account please fill out the Healthy Roster Joining Form.

If you have not received an invitation email from Healthy Roster within a few days of filling out the Healthy Roster Joining Form, please be patient. The Athletic Trainers view and respond to form submissions on a rolling basis, during Athletic Training Hours of Operation only, and will attend to requests as soon as possible. Once your account is created, complete the required documents that have been assigned to you in your profile's documents tabIf no forms are assigned to you, and you are not High-Risk Group A, this step is complete.

  • Tips:
  • → If you don't see an email from Healthy Roster, please check your spam folder
  • → Access codes to create a Healthy Roster profile are unique to each individual and can be found in the email sent to you by Healthy Roster. 
  • → If you are involved in more than one sport, the email you receive from Healthy Roster will only contain one of the sports you are currently involved with. You will not receive more than one invitation email if you are involved in more than one sport. If you would like to check which teams you have been added into, please reference your profile after creating your account.

  • → Refresh the page before accessing
  • To submit your documents after creating your account, scroll to the bottom of the page, click your name under the heading "Athlete Status", and then click on the "Documents" tab within the "Athlete Details" page. High-Risk Group A - Additional Requirements

Men’s Rugby, Women’s Rugby, Ice Hockey & Men’s Lacrosse must complete the following Forms (in addition to the IM Leagues and other Healthy Roster forms listed above). All forms must be submitted by 12 PM three business days before your first practice for you to be marked “Approved” in IMLeagues:

Required Documents
  • Insurance Information Form
  • W&M Personal Health Information (PHI) Authorization Form  
  • VCU Health Personal Health Information (PHI) Authorization Form
  • Medical History Questionnaire OR a Preparticipation Physical Exam (PPE), which is a hard copy signed by a medical professional, then scanned and uploaded to Healthy Roster.
    • The PPE is strongly encouraged, but will not be required.
    • If you do not get a PPE, you must submit a Medical History Questionnaire
    • If the above requirements are not completed and approved before your first activity (practice or competition), you will be suspended from practice and competition immediately and indefinitely until the Sport Club Assistant Director verifies all requirements are met. All forms must be submitted by 12 PM three business days before the practice/game/event. Forms will not be reviewed or approved if submitted within 3 business days of your practice/game/event.
High-Risk Group A - Concussion Baseline Requirement

All High-Risk Group A sport club participants are required to have a valid concussion baseline test, from within the last two years, on file with the Athletic Trainers. If a High-Risk Group A Sport Club participant does not have a valid concussion baseline on file, they will not be able to participate in competition/events until they have completed a valid baseline test.
