Forms are due June 1st for Fall admission and January 10th for Spring admission.
The Health Evaluation Form is an important requirement of all full-time students.
It is important that you complete this document (all immunizations must be current) and return it by the due date of your entrance term.
Failure to complete this requirement by August 31st for Fall admission or February 28th for Spring admission will result in a late fee of $100. Additionally, a late fee of $150 will be charged should this necessary document remain incomplete.
During this process, a hold may also be placed on your Banner account. This will prohibit you from registering for classes. The information required from you is based on Virginia State Law, William & Mary requirements, and information needed to assist in caring for you.
