All full-time students must fulfill both of the following requirements:
1. Health Evaluation Form
The Health Evaluation Form (pdf) provides the Student Health Center with a medical history and official immunization record for each student. It also serves to alert the Health Center to the unique medical needs of each particular student. For students entering William and Mary for the fall semester, the form must be submitted by June 1. For students entering the spring semester, the form must be submitted by January 10.

Nonurgent services will not be rendered without a completed Health Evaluation Form.
Failure to complete this requirement by August 31 for Fall admission or February 28 for Spring admission will result in a late fee of $100. Additionally, a late fee of $150 will be charged should this necessary document remain incomplete.
2. Insurance Requirement
All incoming and continuing, full-time students are required to have health insurance throughout the school year as a condition of enrollment. Every academic year, these students must either elect to enroll in or waive out of the W&M-sponsored Student Insurance Plan to comply with this requirement. Falsifying or misrepresenting insurance information is a violation of the Honor Code and will be reported.
- Students who elect to Waive the Student Insurance Plan because you already have health insurance for the entire academic year must submit an approved Waiver Request by July 31 (12/31 Spring semester). New Students: please note that including a copy of your insurance card with your Health Evaluation Form is NOT sufficient proof of insurance; you MUST submit an online Waiver Request as your proof of adequate health insurance.
- Students who elect to Enroll in the Student Insurance Plan because you are uninsured, underinsured or do not have access to coverage locally must submit an Enrollment Request by July 31 (12/31 Spring semester).
- Students who neither elect nor waive coverage by 7/31 (12/31 Spring semester) will be enrolled in and billed for Student Insurance Plan and will be financially responsible for the Health Insurance Policy charges posted to your Student Account. Please note it is your responsibility to verify that your Student Account is appropriately billed for the Student Insurance Plan and notify us immediately if there is a insurance billing error.
