Use the list below to determine the issue and course of action that needs to be taken. Please call 221-2270 for emergency or urgent work requests. For all others, submit a request using the Facilities Management online maintenance management system:

  • All orders are subject to approval by the Director of EH&S and availability of stock in the warehouse
  • Review and acknowledgement of the Voluntary KN-95 Use Waiver is required before receiving a mask from the university.  Individual purchasers of a KN-95 acknowledge the information in the waiver when they place their order.  Building Managers coordinating bulk orders for a department, unit, or office should circulate the waiver, noting to faculty or staff that opting-in to receive a mask constitutes acknowledgement of the waiver.
  • Please keep all hand sanitizer bottles and pumps for reuse and refill
  • VIMS employees must contact VIMS Facilities Management to request PPE items
