These forms would only be requested if you were seeking need-based financial aid from William & Mary's Office of Student Financial Aid (indicated by your filing of a FAFSA). You do not need to submit any of these forms (including the FAFSA) if you only desire merit-based aid from William & Mary.

If you send unsolicited documents, we are required to verify the information regardless of your verification status. In other words, if you were not originally required to go through the verification process, sending unprompted document will require our office to verify the student's FAFSA data.

Asset Verification Form (fillable pdf form) - For all students selected for the Verification process.
Dependency Status Worksheet (fillable pdf form) - Only submit this form if we have specifically requested it. This form is used to clarify a student's response to questions 47 - 55 on FAFSA.
Homeless Unaccompanied Youth Determination Form (fillable pdf form) - This form is used to clarify a student's response to questions 56 - 58 on FAFSA
Outside Scholarship Reporting Form (fillable pdf form) - Use this form to notify the Financial Aid Office of any scholarships or resources that you have received from organizations or persons outside of William & Mary.
Self Reported Enrollment Plan - FLEX Track, Part-Time, December Graduates, Spring Transfer (fillable pdf form) - Use this form to report anticipated enrollment to your financial aid counselor. Providing us with this form will help expedite your process of review. The goal is to ensure that the financial aid package we create is reflective of your plans and cost and is as accurate as possible.

Appeal for Special Circumstances Consideration (fillable pdf form) - Use this form to report a change in family financial situations not addressed by the FAFSA filing.
Computer Expense/Budget Increase Request (fillable pdf form) - Submit for consideration of a one time budget increase to cover the cost of a computer for the 2019 - 2020 academic year. You can submit this request up to 60 days prior to the start of the fall semester or any time during the academic year. Verification of cost or purchase is required. Approved budget increases will NOT result in additional grant funds, but with loan opportunities.
Dependency Override Form (fillable pdf form) - Only submit this form after consulting with your financial aid counselor.
Dislocated Worker Verification Worksheet (fillable pdf form) - Only submit this form if we have specifically requested it.
Identity and Statement of Educational Purpose (pdf) - Only submit this form if we have specifically requested it.

Original citizenship Document Affidavit (fillable pdf form) - Students required to submit citizenship documents should present these documents in person to the Office of Financial Aid. In cases when this is not possible, complete this form and mail legible copies of your citizenship documents to our office. DO NOT submit this form if you are able to bring your original document to the Financial Aid Office.
PLUS Loan Request Form - Parents (fillable pdf form) - This form is specifically for parents of undergraduate students seeking to have a Parent PLUS loan added to their student's 19 - 20 financial aid award. Parent's may not borrow PLUS loans for graduate student or Independent undergraduate students. It is required that a student has a 19 - 20 FAFSA for a parent to borrow a 19 - 20 PLUS loan.
PLUS Loan Request Form - Graduate Student (fillable pdf form) - This form is specifically for graduate students seeking to have a Graduate PLUS loan added to their 19 - 20 financial aid award. It is required that a student has a 19 - 20 FAFSA on file to borrow a 19 - 20 PLUS loan.
Sibling Enrollment Verification Form (pdf) - Only submit this form if we have specifically requested it. This form must be completed and returned to us by sibling's home institution.
Summer Financial Aid Application (fillable pdf form) - Use this to inform your financial aid counselor of your summer enrollment plans.
