ITA - Nominate Someone to the Hall of Fame

If you would like to nominate someone to the ITA Women's Tennis Hall of Fame, we will need the following information:

	The name of the nominee and her current address, telephone number, fax number, and email address
	A copy of the nominee's resume
	Other background information about the nominee including photographs if possible (or where to get them) - we will return all photos
	The college(s) that the nominee attended and graduated from, including the year she graduated
	The nature of your relationship to, or association with, the nominee
	A signed statement, including your contact information, as to why you think your nominee should be included in the ITA Women's Tennis Hall of Fame
