Forms

The following forms are available in the Sharpe Office, Tucker Hall, Room B2A and in the Charles Center in Tucker basement:

Unless otherwise indicated, all forms should be returned to the Charles Center, Tucker Hall, Room B-2.

Service-Learning Courses

All students wishing to register for service-learning courses are responsible for reading and following the instructions for each form. Delays may result if forms are not completed properly.

Community Scholars House Participants in Service-Learning

Students are required to approach faculty sponsors to evaluate project proposals as part of their application.

Current CSH students must complete an Independent Study Form before the end of the add/ drop period of the Spring Semester.
