We understand that information about our insured students' health is personal and we are committed to protecting that information. In fact, we are required by law to protect the privacy of your health information. Protected Health Information (PHI) includes any information we maintain that reasonably can be used to identify the insured and that relateds to the insured's physical and mental health conditions, the provision of health care to our insureds and the payment for such health care. We cannot disclose your protected health information to anyone, other than you including members of your family or your parents, unless we have your written authorization to do so. This is true EVEN if the insured student has signed the W&M Parental Information Release Form from the Dean of Students office.
To authorize a parent or another individual to act on your behalf for health insurance matters with UnitedHealthcare StudentResources (UHCSR), the insured student may login to MyAccount and complete the Personal Representative Appointment (PRA) in the Forms and Letters section of the home screen. Alternatively, the insured student may complete and return by fax the Personal Representative Appointment form. Please note that the W&M Student Insurance Plan is a one-year, term policy and expires each year on 7/31, therefore this PRA will expire at the end of the current policy year. Insured students must file a new PRA each policy year.
