Print and deliver these completed documents and original forms of ID (US Passport OR Social Security Card & Driver's License) to the Rec Sports Office at 400 Brooks Street, Williamsburg, VA 23185 with attention to Joe Tighe, Assistant Director of Intramural Sports or Daniel Gardner, Graduate Assistant of Intramural Sports.
