Student Insurance Waiver Information

All full-time undergraduate and graduate students admitted Fall 2006 or after will be enrolled in the college-endorsed Student Health Insurance Plan and the cost will be billed to your student account in two installments (fall and spring semester) UNLESS proof of other adequate health insurance coverage is furnished. Students who already have health insurance for the entire academic year must submit a waiver request by the deadline each academic year, and the waiver request must be approved to avoid being enrolled in the Student Insurance Plan. Waiving the Student Insurance Plan does not affect eligibility for services at the Student Health Center. Students with limited out-of-area coverage (HMOs) or otherwise inadequate coverage (HDHPs) are urged to carefully review their options before waiving the College-sponsored insurance plan.

You will need your unique 9-digit W&M Student ID number (beginning with 93) and current insurance information. Click on the link under the appropriate student heading below to access the Waiver Request form.

Important Note: You will be redirected to a secure waiver system provided by the contracted student insurance company, UnitedHealthcare.  Your private insurance information is secure and can only be accessed by a College of William & Mary administrator with a unique logon and password. The form requires a simple, electronic signature, not an encrypted, digital signature.  You only need to type your name onto the form to indicate that you accept the acknowledgement. Please include reference initials if someone other than the student completes the form. Immediately after submission, you will receive an onscreen and email confirmation sent to the email address supplied on form. You should retain a copy of one of these confirmations for your records. Students will be responsible for the charge(s) on their student account if the Waiver Request is not approved.

Deadlines

To avoid being billed for the Spring/Summer coverage of the Student Insurance, a 2011-2012 Waiver Request is due by January 10 for all new, transfer or readmitted students entering the College Spring 2012 AND all continuing students who did not file a 2011-2012 Waiver Request in a timely manner in the Fall 2011 term.

Waiver Requests submitted after the waiver period has ended will be considered at the discretion of the College and on a case-by-case basis. To be considered after the waiver period, you must submit a 2011-2012 Notice of Change of Insurance for review to determine if your request to be waived from the Student Insurance will be approved. If your request is approved and you were charged, you should see the credit post to your Banner Student Account Statement & Payment History within 3-5 business days. If you request is not approved, you will be responsible for the health insurance policy charge on your student account.

Domestic Students

Insurance plans submitted for waiver must provide coverage for the remainder of 2011-2012 academic year.  This is a mandatory requirement.

If your plan meets this requirement, you should submit a Domestic Student Spring 2012 Waiver Request in a timely manner for consideration.

F-1 & J-1 International Students

Insurance plans submitted for waiver must provide coverage for ALL of the following requirements: (see helpful Guidelines for more information)

  1. Coverage for the remainder of the 2011-2012 academic year
  2. Coverage of at least $75,000 per injury or sickness
  3. Coverage of at least $7,500 for Repatriation and of at least $10,000 for Medical Evacuation
  4. Coverage for mental health (including suicide attempt, substance and alcohol abuse treatment)
  5. Coverage for harm to self, suicide or attempted suicide
  6. Coverage for maternity/pregnancy care (females only)

If your plan meets ALL of these requirements, you should submit an International Student Spring 2011 Waiver Request in a timely manner for consideration. If your plan DOES NOT meet all of the requirements or you cannot provide proof of the same, you WILL NOT obtain a waiver, so please do not submit a request for one. F-1 & J-1 International Students should not purchase any other health insurance until they are certain their waiver request will be approved.
