Full Description
All incoming full-time students must fulfill both of the following Student Health Center requirements:
The Health Evaluation Form (pdf) provides the Student Health Center with a medical history and official immunization record for each student. It also serves to alert the Health Center to the unique medical needs of each particular student. For students entering William and Mary for the fall semester, the form must be submitted by July 1. For students entering the spring semester, the form must be submitted by January 10.

The Health Evaluation Form must be completed in full and received by the Student Health Center by the due date for your entering semester to avoid possible judicial action.
2. Insurance Requirement & Waiver Request
All incoming full-time students will be enrolled in and billed for the College-sponsored Student Insurance Plan UNLESS proof of other adequate health insurance is furnished. Students who already have health insurance for the entire 2012-2013 academic year must submit a Waiver Request by July 1 and by the posted deadline EACH YEAR thereafter. The 2012-2013 Waiver Request will be available between 5/15-6/1. Please note that including a copy of your insurance card with your Health Evaluation Form is NOT sufficient proof of insurance; you MUST submit a Waiver Request as your proof of adequate health insurance.
