Reporting a Possible Violation/Incident
Students, faculty, and staff may submit reports using our online reporting form. This form may be used to report a possible conduct, honor, or hazing incident. Faculty members may also use the form to report general student welfare concerns or class absences.
You must log in using your WM ID and password (the same ID and password as your myWM account).
If you are not a student, faculty, or staff member, you may use the following forms to report possible violations. When you have completed the form, please print it out and bring it to the Office of the Dean of Students (107 Campus Center) or fax it to 757-221-2538. Please feel free to call the Dean of Students Office (221-2509) if you have any questions regarding reporting a violation.
