Reporting a Possible Violaition:

Please use the following forms to report possible violations. When you have completed the form, please print it out and bring it to the Office of the Dean of Students (109 Campus Center) or fax it to 757-221-2538.  Please feel free to call the Dean of Students Office (221-2509) if you have any questions regarding reporting a violation.
