Student Form

Thinking about becoming a Neuroscience major? Our program is an Interdisciplinary Program that is administered through the Charles Center. To declare that you are a Neuroscience major, you must complete a form:

Submit the form along with a copy of your current degree evaluation to the Registrar.

You must get the form signed by the Program Director (John Griffin) or Associate Director (Pam Hunt), or by a Neuroscience Advisor who has agreed to act as your academic advisor.
