Forms for Student Employment

1. Initiation Form (pdf):  This form is required for setting the student up in the payroll system. On this form, your supervisor provides the start date, pay rate, account number, and departmental information. The form is then forwarded to Financial Aid for processing. You will also use the Initiation Form for any pay-rate adjustments or to terminate employment.

2. I-9 Form - Immigration/Naturalization Form (pdf): Verifies U.S. citizenship. The student will need to provide documentation such as a Social Security card and drivers license or a U.S. Passport. After completion, it is forwarded to Financial Aid for processing. Please do NOT turn this form in now for students who won't be starting until the Fall.

3. Tax Forms (State and Federal) & Direct Deposit Authorization Forms: Given by department to student and go to payroll office.
