Below is a complete list of the Forms:

 

Committee on Academic Status (CAS)

    The Form for Petition is a request for exception to College regulations.  Exception is generally only granted in extraordinary circumstances.  A student should never assume changes to their current schedule without written authorization of Committee approval.

    The following is a list that this form covers:

    Medical petition, personal withdrawal, drop a course, late add a course, overload, underload, change a class from graded to pass/fail, change a pass/fail to graded, class withdrawals, late registration for financial reinstatement.

Dean Certification Process

Students who are transferring, applying to professional schools, sitting for bar exams, or needing information communicated to others about their performance may need official forms completed on their behalf. The Office of the Dean of Students may complete certain sections of Dean Certification forms for students. Forms that request class rank, cumulative grade point average, or degree information must also be completed by the Registrar’s Office. Click here for more information about this process.

Declaration of Disability

The College of William and Mary welcomes a widely diverse population of students including students with disabilities.  In order to provide an accessible educational environment and to meet the individual learning needs of students, we invite self-declaration of disabilities to the College’s Assistant Dean of Students for Disability Services. 

All information submitted regarding disability will be handled confidentially and is shared only with express written permission for accommodation purposes.  The College cannot provide accommodations that are not requested. 

After enrolling at the College, students are expected to meet with the Assistant Dean of Students for Disability Services after attending all classes at least once and within the first three weeks of the semester to discuss current course load and propriate accommodation(s).  Letters authorizing accommodation are prepared and ready for pick up within 48 hours of this initial  meeting.  They must be hand delivered to professors in a timely fashion (usually within the week of pick up) and this  procedure is repeated each semester that the student is in attendance.

Deferred/Rescheduled Exams

    The Dean of Students Office only has the authority to DEFER examinations.  Deferred exams are given during the make-up period at the beginning of the following semester.  If you would like to request a change in your exam time, you must go to the Dean of Undergraduate Studies Office in Ewell 123.

Medical Clearance and Release of Information

    All students who need to authorize representatives of the Dean of Students Office, Counseling Center, and Student Health Center to have release/exchange of information to/with a medical professional must have a current release form on file.

Parental Information Release and Parent/Spouse/Guardian Information Update Forms

If students want to change the status of their parental release information, they must fill out a new Parental Information Release Form.  It must be printed out and signed. The form can either be mailed, faxed or brought in person to the Dean of Students Office.

The Parent/Spouse/Guardian Information Update Form should be used by students to update the College and William and Mary on contact information for their parents, guardians or spouse.  This form also includes the Parental Information Release.

Readmission Form


Students Called to Active Duty in the Military Services of the United States

This statement describes the ways in which students whose active military duty during a time of national emergency requires sudden withdrawal or prolonged absence from the College of William and Mary may seek tuition relief, refund of payments, and reinstatement.

Restricted Parking for Medical Reasons

Freshman and sophomore students seeking access to a personal vehicle and restricted parking for medical reasons are asking for an exception to College policy. All such exceptions must be reviewed and recommended to Parking Services by the Medical Review Committee (MRC). Petition approval is not automatic and is given only when a student’s medical appointments cannot be accommodated by the William and Mary (or greater Williamsburg) Transit Service.

The request for restricted parking for medical reasons is a two part petition process. Form A must be completed by the student seeking access to his/her personal vehicle. Form B must be completed by an appropriate medical professional who will be working with the student and who is not also a member of the student’s immediate family.

The Medical Review Committee, in consultation with Parking Services, reserves the right to review all documentation and recommendations from outside experts to determine eligibility and to deny any request which seems insubstantially supported, excessive, or unnecessary. Please submit the petition and all supporting medical documents at least two weeks before the vehicle will be needed. Vehicles parked on campus prior to receiving permission through this procedure will be ticketed appropriately.

Temporary Handicapped Parking

This form is for use by physicians working with William and Mary students who are currently enrolled and experiencing mobility limitations which cannot otherwise be accommodated by the William and Mary transit service. Temporary permits are valid on campus only and are issued only when students have access to a personal or family vehicle. In general, temporary permits do not exceed ten business days. Documentation for handicapped parking permits cannot be accepted from a member of the student’s immediate family. The Dean of Students Office reserves the right to review all documentation and recommendations from outside experts to determine eligibility and to deny any permit request which seems insubstantially supported, excessive, or unnecessary.

Notification of Absence from Campus

Except for reasonable cause, students are expected to be present at all regularly scheduled class meetings. Students who miss classes due to a death of an immediate family member, extended medically related absences (more than 1 or 2 days), general family emergencies, personal difficulty and campus crises should fill out the form below. Forms should be submitted to the Office of the Dean of Students, 109 Campus Center or faxed to (757) 221-2538. Letters will be seant via campus mail to professors; this process typically takes 1-5 days depending upon individual circumstances.
