    • COLL 100 Section Application Form. Instructors should use this form to have a specific section of COLL 100 approved (ENSP 100-01, COLL 100-01, PHYS 100-01, etc.). Be sure to submit a syllabus separately to [[epc]]. Want to see samples?
    • COLL 150 Section Application Form. Instructors should use this form to have the writing component of a specific First-Year Seminar approved (e.g., ENSP 150-01, ENGL 150-01, etc.). The writing component will be reviewed by the Writing Committee the first time that section is offered as a COLL 150. Instructors must receive approval for the course even if it was previously approved as a 150W. More information here.
    • COLL 200 Application Form. This form is to seek COLL 200 approval for a new or existing course. Be sure to also submit a syllabus to [[epc]]If this is a new course, you probably won’t need to submit a New Course Application Form. Want to see samples
    • COLL 300 Application Form. Be sure to also submit a syllabus to [[epc]]. This form is to seek COLL 300 approval for a new or existing course. It is intended for faculty-led study-away experiences. It is not intended at this time for COLL 300 Colloquia. If this is a new course, you probably won’t need to submit a New Course Application Form. 
    • COLL 400 Application Form. Be sure to also submit a syllabus to [[epc]]. This form is to seek COLL 400 approval for an existing course. If this is a new course, you should first submit a New Course Application Form (below). 
    • Active Learning Application Form. This form is to seek Active Learning approval for an existing course. If this is a new course, you should first submit a New Course Application Form (below)
    • Domain Credit Application Form. This form is to seek ALV, CSI, or NQR domain credit for an existing course. If this is a new course, you should first submit a New Course Application Form (below)
  • New Course Application Form. Please use this when you are creating a new course number and catalog language. If this is a new COLL 100, 150, 200, or 300, the forms above may suffice.
  • Course Change Form. Please use this when you are making changes to an existing course (changing the number of credits, changing the catalog language, changing the prerequisites, or asking for a cross-list with an existing course).
  • Curriculum Change Application Form. Please use this form to change catalog language associated with a major or minor.
